Action Required: 2025 NCIP Provider Profile March 2025

North Carolina Immunization Program
Mar 3, 2025

The North Carolina Immunization Program (NCIP) provider profile submission is now open. All NCIP enrolled providers are required to submit a provider profile (which indicates the number of eligible and non-eligible patients served by your practice) every 12 months to continue to receive vaccine(s) and remain active in the program. Failure to complete the provider profile survey by April 4, 2025 may result in suspension from the NCIP.

The Provider Profile must be completed online at this link.

What You’ll Need to Complete the Provider Profile:

  1. Practice information
  2. Patient population (profile) numbers
  • NCIR benchmark data is available within the survey; however, this should only be used as a tool to assist providers in determining their numbers.
  • Your enrollment numbers should represent the number of patients being served by age group and eligibility category who received a vaccine between March 1, 2024, and February 28, 2025. Count a child only once based on the age/eligibility categories at the last immunization visit, regardless of the number of visits made. Accuracy in reporting these numbers is a critical part of determining the correct vaccine supply for your eligible patients.

       3. Primary and back-up coordinator contact and training information.

The primary and back-up coordinators are required to complete annual training. If annual training (within the past 12 months) has not yet been done – it's not too late! Please complete your annual training by completing both of the modules linked below. Continuing education (CE) credit is offered for each module.

a. CDC’s 2025 You Call the Shots: Vaccine for Children (VFC) - Module 16

b. CDC’s 2025 You Call the Shots: Vaccine Storage and Handling – Module 10

  • In lieu of completing the modules, experienced healthcare professionals familiar with VFC and federal storage and handling requirements can opt to take a refresher test to demonstrate their knowledge. Note: No continuing education credits will be awarded for completing either refresher test. To save your survey and finish later:

You must save this code in order to return to your saved survey. When you’re ready to return to your survey, select “Returning?” in the upper right corner to enter your code.

*If you are completing the profile for multiple locations, you must begin a new survey for each site. Returning to a previous survey may overwrite the previous submission.
