  Complete a preliminary enrollment guide.

  Complete and submit the application packet

  After you submit the preliminary enrollment guide, you'll get an email with an application packet and more information about enrollment.

  Submit your completed application, along with these documents:

  • One week of documented in-range temperature logs. You'll need to document temperatures once a day, including the min/max each morning.
    • Refrigerator Temperature Log with Min/Max (PDF)
    • Freezer Temperature Log with Min/Max (PDF)
  • Certificates of calibration for each digital data logger thermometer. You'll need an approved digital data logger on each unit as well as a back-up. How to use a data logger (PPT).
  • Training certificates for primary and back-up vaccine coordinators (two certificates each)
  • Grant notification or certificate (only applicable to FQHCs, RHCs, and Community Health Centers)

  Review and submit the 317 VFA Provider Agreement Contract

  Once approved, you'll get the NC 317 VFA Provider Agreement (contract). You'll also get templates for the Routine and Emergency Vaccine Management Plans.

  Please submit all, with the agreement signed by the lead physician.
