How to Become a 317 Vaccines for Adults (VFA) Provider in NC

The North Carolina Immunization Program (NCIP) supports immunization of uninsured and underinsured adults and other priority populations through the 317 Vaccines for Adults (VFA) Program. To become a VFA provider, you'll need to join NCIP by following the steps below.

Step-by-Step Guide to Join NCIP

    Confirm you serve eligible adult populations.

    Review information about the 317 Vaccines for Adults (VFA) program and commonly asked questions.

    Review NCIP coverage criteria

    Complete a preliminary enrollment guide.

    Complete and submit the application packet

    After you submit the preliminary enrollment guide, you'll get an email with an application packet and more information about enrollment.

    Submit your completed application, along with these documents:

    Review and submit the 317 VFA Provider Agreement Contract

    Once approved, you'll get the NC 317 VFA Provider Agreement (contract). You'll also get templates for the Routine and Emergency Vaccine Management Plans.

    Please submit all, with the agreement signed by the lead physician.
