I understand that by using my Well Contractor License/Certificate from the state indicated above to obtain NC Well Contractors Certification at my desired level of certification that it is my personal and professional responsibility to read and familiarize myself with all of the NC Laws and Rules and any local county or other municipality rules in effect.

I understand that failure to adhere to the NC Laws and Rules when conducting well contractor activities in the state of North Carolina may result in the issuance of civil penalties from the applicable agency, and/or disciplinary action by the North Carolina Well Contractors Certification Commission.
