Please complete this form to request to take the NC Well Contractors Certification Exam. Check upcoming exam dates.

We must receive your completed form and payment at least 10 business days before the exam. Your exam date must be at least 30 days after approval of your certification application.

A new application is required if you do not pass the exam within three consecutive attempts or within one year of submitting your application, whichever comes first.
