The purpose of this memo is to remind local health department (LHD) staff about the upcoming 2025 Annual Immunization Assessment. This assessment will review NCIR records of children, 24 through 35 months old, who are labeled "active" in NCIR under the LHD site, as well as all children that live in the county.
The goal of this assessment is to determine the percentage of children who are documented as up to date (UTD) on recommended vaccines by age 24 months. Clients with a birth date range of 10/2/2022 through 10/1/2023 will be assessed for the 4:3:1:3:3:1:4 (4 DTaP, 3 Polio, 1 MMR, 3 Hib, 3 Hep B, 1 Var, and 4 PCV) series.
Data will be extracted from NCIR on Tuesday, October 1st, 2025. Please note some children may be excluded (made “inactive”) from your LHD assessment rate. Please refer to the attached document titled, “Exclusion Criteria for Making Clients Inactive from an Organization” for more information.
Assessment results will be reviewed with you in early 2026 by your Regional Immunization Consultant (RIC). These rates are referenced as performance measures in the 715 LHD Agreement Addenda. If you have any questions regarding this memo or the assessment process, contact your RIC.
