North Carolina Immunization Program

This is a reminder that annual reports for all North Carolina public, private, and religious schools, including charter schools, are due by November 1, 2025.
Requirements:
- Public and Charter Schools: Submit Health Assessments for students enrolling for the first time (§ 130A-441(c)) (PDF).
- All Schools: Report immunization status for all students in kindergarten, seventh, and twelfth grades (G.S. 130A-155(c)) (PDF).
Note: This communication is being sent to all schools, if you have already completed your report no action is needed.
How to Submit the Report
- The reporting portal is now open, and reports may be completed at any time prior to the due date.
- Students must meet all requirements within 30 calendar days from their first day of school attendance.
- Compliance reports are due no later than Nov. 1, 2025.
- Questions about Health Assessments and reporting may be directed to your Regional School Health Nurse Consultant.
- Questions about Immunization Reports can be emailed to immunization.reports@dhhs.nc.gov.
