Dear Providers,
NCIP provider recertification is now open. Providers must complete recertification online through the 2026 North Carolina VFA Provider Agreement by April 25, 2026.
To stay active and continue receiving vaccines, all enrolled providers are required to complete:
- Recertification at least once every 24 months
- Provider profile submission every 12 months, including the number of VFC-eligible and non-VFC-eligible patients served by your practice (included in this year’s recertification)
Note: Submit only one agreement per location and retain a copy for your records. Paper copies will not be accepted.

What You'll Need to Complete Recertification
- Practice information
- All practicing provider’s information, including medical license and NPI numbers
- Patient population (profile) numbers
o NCIR benchmark data is available within the survey; however, this should only be used as a tool to assist providers in determining their numbers.
o Your enrollment numbers should represent the number of adults being served by age group and eligibility category who received a vaccine between January 1, 2025, and December 31, 2025. Count an adult only once based on the age/eligibility categories at the last immunization visit, regardless of the number of visits made. Accuracy in reporting these
numbers is a critical part of determining the correct vaccine supply for your VFA-eligible patients. - Primary and back-up coordinator contact and training information
o The primary and back-up coordinators are required to complete annual training. If annual training (within the past 12 months) has not yet been done – it's not too late! Please complete your annual training by completing both of the modules linked below:

o CDC’s 2026 You Call the Shots: Vaccine Storage and Handling – Module 10
- In lieu of completing the modules, experienced healthcare professionals familiar with VFC and federal storage and handling requirements can opt to take refresher tests to demonstrate their knowledge. Note: No continuing education credits will be awarded for completing either refresher test.

Lead provider (MD, DO, NP, PA, or RPh) must be available to provide their signatures.
To save your survey and finish later:
If you need to save the document and return to it later, select “Save & Return Later” at the bottom of the survey and you will be given a return code. You must save this code in order to return to your saved survey. When you’re ready to return to your survey, select “Returning?” in the upper right corner to enter your code.

Additional Requirements and Feedback
Sign your annual NCIR confidentiality form, and keep a copy for your records. These do not need to be submitted to the NCIP.
A section for your feedback is included at the end of your recertification and must be completed before submission. We encourage you to reflect on your experience with our program over the past year and share your input to help us improve and better serve both you and your patients.
