Dear Local Health Departments,
The purpose of this memo is to remind local health department (LHD) staff about the upcoming 2026 Annual Immunization Assessment. This assessment will review NCIR records of children, 24 through 35 months old, who are active in each LHD and those who live in each county.
The purpose of this assessment is to determine the percent of children who have documentation of being up to date (UTD) on recommended vaccines by age 24 months. All active clients with a birth date range of 10/2/2023 through 10/1/2024 will be assessed for the 4:3:1:3:3:1:4 (4 DTaP, 3 Polio, 1 MMR, 3 Hib, 3 Hep B, 1 Var, and 4 PCV) series.
Data will be extracted from the NCIR on Thursday, October 1, 2026. These rates are referenced as performance measures in the 715 LHD Agreement Addenda. Please note some children may be excluded (made “inactive”) from your LHD assessment rate. Data from this assessment will be reviewed with you in early 2027 by your Regional Immunization Consultant (RIC). If you have any questions regarding this memo or the assessment process, contact your RIC.
