Health Care

  • We work with health care providers to encourage a team-based care approach for hypertension and cholesterol management by promoting self-measured blood pressure monitoring with clinical support for adults with hypertension.
  • We encourage health care providers to use electronic health records to identify and address health disparities.
  • We facilitate the multi-disciplinary North Carolina Cardiovascular Health (CVH) Learning Collaborative (LC) with the goals of identifying and addressing barriers to social services and supports within populations at highest risk of Cardiovascular Disease (CVD).
  • We promote the adoption of evidence-based quality measurement at the provider level to monitor health care disparities and implement activities to eliminate those disparities.
  • We provide support in creating and enhancing community and clinical linkages to identify and address Social Determinants of Health (SDoH) of populations at highest risk of CVD.

Community

  • We provide tools and resources to persons with hypertension to encourage self-measured blood pressure with clinical support.
  • We promote education materials that empower people at risk of or who have heart disease to work with their health care team to set and reach hypertension goals.
  • We support community pharmacists to promote SMBP (Self-monitored Blood Pressure) for the purpose of managing high blood pressure.
