Helping local health departments reduce chronic disease and injury

The Healthy Communities Program works with local health departments to reduce the burden of chronic disease and injury in our state. The program is administered through the NCDHHS Division of Public Health Chronic Disease and Injury Section using Preventive Health and Health Services Block Grant funding.

This funding helps local health departments implement community-based strategies that address poor nutrition, physical inactivity, tobacco use, violence and unintentional injury.
