Syringe Services Program Sign-Up Form

Prior to commencing operations, NC SSPs are required to register with the NCDHHS Division of Public Health (DPH), by completing and submitting the Syringe Services Program Sign-up Form and sending additional materials to SyringeExchangeNC@dhhs.nc.gov.
